How can this happen again?

One would think a Admiral would make damn well sure all his captains under his command knew that ship safety is the #1 priority.

I could have written the first one off to poor command on the ship, this is starting to look like poor command at the admiral level.

My guess is that the fact that both of these Destroyers were part of the 7th Fleet, which is the fleet based out of Japan. In case you don't know, the 7th fleet is the front line in any conflict with NK or China. That means that most likely the Commander of the 7th fleet will be relieved of command. You can't lose 25% of the Destroyers under your command during peace time and keep your job.

Really weird.
There are watches all around, especially in heavy traffic.
Both ships are large and cannot suddenly move to strike the smaller ships.
So the smaller ships both got underfoot of the larger ships.
I have seen small cargo ships dart under the bow of our carrier.
They are missed by a few feet.
These accidents are the opposite as they are the small ships.

I installed a CARD (collision avoidance radar) on my 35 ft sailboat for $2,000. I set the perimeter about 6 miles out when in open seas and an alarm would sound if anything wandered into that radius. I thing the navy can afford 2k. I'd install it, but I have shit to do.

They have far more than that as well as people on watch.
That is what is so weird about both collisions.
Damn sure was not the large ships swerving.
They do not exactly swerve.

Like the Fitz, the Captain, the XO, the Command CPO will lose their jobs no matter what is found for the collision. Probably a few junior officers and leading PO's on the watch duty will get administrative punishment. For all these people their career is effectively over, and rightly so.

I heard the McCain lost it's underway power or steerage (can't remember which, or if both). There are emergency procedures to follow, and ya gotta implement them quickly, especially in a heavy traffic region like this strait.
Yet the McCain sailed into port under it's own power...that makes me wonder.
