AVs scan for binary patterns and names, good ones have an HIPS. Norton with SONAR enabled is the only one I've ever seen that actually caught non-noob malware.

Run your browser inside sandboxie with passwords master-key encrypted. Same for warez and questionable software.

It also helps to run an ARK occasionally, RKU latest on 32bit and SanityCheck or Avast ARK Beta on 64bit. Rootkits do bypass signing and patch-guard on 64bit despite what 'experts' say, there are at least 3 ways to do it.
